The Tattoo Consent Form (Canada) is a consent form template to be used by tattoo parlors before a tattoo artist performs any work on a client.

The form states that the client has had the opportunity to ask any questions pertaining to the obtaining of a tattoo prior to the work being performed, and that all facts about the procedure have been explained. There are a number of points that the client accepts and signs to confirm, including revealing any conditions that would contraindicate having the tattoo (pregnancy, nursing, being under the influence of drugs or alcohol), that the client does not have any medical conditions or skin conditions in the area of the tattoo, and that infection is always a possibility. There is acknowledgment that variation in color or design may exist, and that obtaining a tattoo is a permanent change to the client's appearance.
