The full details of the project including the project name, project number, contractor, architect, and escrow agreement number. The contractor must submit the contractor with the approval from the construction administrator to the State Treasurer’s Office.
All data from the Contractors Payment Request form must be submitted on the form and approved by the construction administrator. The amounts for the total contract, total earned to reference payment, and total retention to date should all be calculated to get to the final total retention payment.
